This document holds certain words with capital letters, which have been defined here for your clarification:
“SJ EMR” is a service provider that stores data of healthcare seekers and healthcare service providers. "Content" means text, graphics, images and any other material entered, processed, contained on or accessed through the applications/features, including content created or modified. "Services" means services provided through the SJ EMR application.
SJ EMR Contents and Services
SJ EMR offers both free and paid services. SJ EMR has a monthly subscription after 2 months of free trial. Our subscription plan is only for doctors.
SJ EMR is mainly for personal use. Its services offer individual support for member’s data storage.
Membership Terms and Termination
After accepting the proposal, you can start enjoying the privileges of SJ EMR membership free for 2 months. You may cancel your membership at any time. Please email or contact us by calling at +8801707074577 or +8809611677336, if you wish to cancel your membership. Your membership benefits are not transferable and are for your own personal use only. SJ EMR reserves the right to cancel your membership at any time.
What is SJ EMR?
SJ EMR or SJ Electronic Medical Record is a digital version of information such as medical history, diagnosis, medication, vaccination date, allergies, lab results, and doctor's notes that you would normally find on a doctor's paper chart. This software is used by hospitals, clinics or individual physicians to record patient information and find it quickly at any time to ensure the diagnosis and good quality treatment of the patient through the hospital of that physician.
Doctor-Patient Relationship
The use of SJ EMR will not create a doctor-patient relationship; however, it will help doctors to make fast and reliable decisions. SJ EMR contains members limited personal contents that may help a member identify services or the AI of the application may offer services to the member. Everything on SJ EMR, including contents accessed or services provided through us, are for your information and usage, which should be used by users for informational purposes only.
We are not your Doctors
Whenever we use the words “your doctor” or “Healthcare provider” or similar words on SJ EMR, including these terms of use, we mean your personal doctor within our panel, with whom you have mutually acknowledged doctor-patient relationship.

Membership Renewal
Membership renewal must be made before the expiration of one’s membership. The renewal fee is the same and payment will be accepted via the same means as before to continue receiving the benefits of the membership program. Members who fail to renew their membership will not be able to access and use the benefits of their premium membership.
Un-subscription
Subscriber at any point during the month may decide to unsubscribe. We will not return a refund for the features already paid for. Unsubscribed members, at any point may wish to subscribe back. They will again be charged the same amount for the features that they purchase.
Member's Responsibility
As a member of SJ EMR program you have the following responsibilities: Review and understand the information you receive about your data storage benefit plan. Please call ‘SJ EMR Office’ or contact us via email, when you have questions or concerns. Understand how to obtain services and supplies that are covered under your plan. Pay all fees for which you are responsible at the time of service renewal/render or when the payment is due. Voice your opinions, concerns or complaints to SJ EMR customer service.
Members’ Personal Health Record (e-PHR)
Good medical records are an indication of good practice and good patient care. Member's of SJ EMR should use it's features to record medical details. In managing your patient's healthcare records the following tips as below, would be useful:
Electronically recording all your healthcare information requires a little effort at first, but you’ll be amazed at the difference it makes at every appointment and pleased at how easy it gets tracking your patient's health data. Once you have keyed in all your patient's health data, each update takes a very short span of time and is well worth it, compared to the frustration of not having a desired medical report when needed.
Make sure every new patient you visit is given a copy of their healthcare record which is easily printable from your member’s portal. Get copies of blood and other tests done and update these results in your member’s portal immediately. Ensure that you take great care in updating your patient's healthcare results to ensure accuracy. If you have doubts or need clarification, call us at SJ EMR.
Your use have to be for legal purpose
SJ EMR will use your personal information in a secured manner. SJ EMR has implemented various encryption and security technologies and some procedures to protect you from unauthorized access. You are completely responsible for the access of your username and password. SJ EMR reserves the right to revoke or deactivate your username and password at any time.
Acceptance of these Terms
Generally, these terms and conditions govern the use of this application. Other terms may apply to your use of a specific feature or parts of our application. If there is a conflict between these TOU (Terms of Use) and terms posted for a specific feature(s) or part(s), the latter terms shall apply to your use of that feature(s) or part(s).
